Background
The Innovation Company has created a simple, two question survey that is based in research done by leading academic institutions to measure the level of innovation within a team or company. The two questions focus on the level of risk an employee is willing to take and his or her perception of what would happen if he or she failed after taking that risk. Background

What are the goals and objectives?
- Obtain feedback from teams and groups on the survey and the overall process
- Test the analytics process for accuracy
- Provide teams or companies with information that could be valuable to their organization
What is needed from the participating organization?
- A sample size of at least 12 people, where the ratio of associates to managers would ideally be 4:1 (ideally one manager for every four associates).
- An agreement to send an email to all employees participating in the survey process asking for a response within 1 day.
- The ability to devote 30 minutes after the data is collected and analyzed to review the information and share feedback with The Innovation Company.
What will The Innovation Company provide?
- A template for the email that can be sent to survey participants with the survey link. This can be edited as needed by the participating organization.
- A report that contains the analysis of the data.
- A free POP! Culture® innovation workshop in the Boston area or 2 hours of consulting time to the participating organization in exchange for taking the time to assist with this process.
