The Innovation Company has created a simple, three question survey that is based in research done by leading academic institutions to measure the level of psychological safety within a team or company. The three questions focus on the level of risk an employee is willing to take and his or her perception of what would happen if he or she failed after taking that risk.

Here's how the process works.
1. When you click the NEXT button below you will be taken to a sign-up page via a new browser window.
You can also access the page directly at http://survey.innovationiseasy.com
2. On the new page you will create an Administrator account by entering your email and creating a password.
Your email will be your user name. Note: You can only have one account per email.
3. Once you create your account two emails will be sent to you from survey@innovationiseasy.com.
a. The first email will contain a link to your Administrator Console where you can manage your survey(s).
b. The second email will contain a link that you can use to distribute to the people you wish to take the survey. This is a unique link and anyone who responds to the survey via this link will be included in your results.
4. Distribute the link to the people who you wish to take the survey.
You can simply forward the email with the survey link to the participants. To make this simple for you the email will contain text that you can edit or modify as you see fit. How easy is that?
5. You can log into your account at any time to view how many people have responded and to also close the survey. You can access your Administrator Console at http://survey.innovationiseasy.com/manage_surveys. You may want to bookmark this page.
6. Once you close the survey an automatic report will be sent to you in a PDF format that you can view and print.
The report will provide you with summary data as well as a score that is generated using our proprietary I3 Assessment algorithm. There will also be links with ways to contact The Innovation Company.
